The Commissioner for Human Rights (CHR), when designing its website https://formularze.brpo.gov.pl, hereinafter referred to as the CHR Website, attaches special importance to respecting the privacy of its Users (visitors to this Website). The default settings used on the CHR Website do not allow the collection of any data other than that necessary for the operation of this website and are used solely for administering and ensuring its proper functioning. It does not identify the User nor does it transfer any information about Users to third parties.
The Operator of the Website is the Commissioner for Human Rights, with its registered office at Al. Solidarności 77, 00-090 Warsaw.
Data Collection
The CHR Website stores only the HTTP requests directed to the CHR server, and the content viewed is identified by URLs. Such collected data, stored in log files, may include the following information:
- the public IP address of the computer from which the request originated (this may be the User’s computer directly),
- the time the request was received,
- the first line of the HTTP request,
- the HTTP response code,
- the number of bytes sent by the server,
- the URL of the page previously visited by the User (referer link) – in cases where access to the CHR Website occurred via a link,
- information about the User’s browser,
- information about any errors that occurred during the HTTP transaction.
Cookies Mechanism
“Cookies” are a type of small data file – teleinformation data – that are sent by every website visited by a User and are stored on their terminal device, for example on a computer or smartphone used for browsing web pages.
On the CHR Website, this information is used solely to tailor the website content to the User’s preferences, in particular to recognize the User’s device and display the website appropriately, adapted to their individual needs. These include:
- visid_incap_xxx and incap_ses_xxx (essential types) – used to protect the website against threats such as DDoS (Distributed Denial of Service) attacks and other types of unauthorized traffic. The files are provided by a company responsible for security.
- SESSxxx (essential type) – stores the session identifier (exclusively for CHR editors and administrators); it is stored on the terminal device for 23 days or until manually deleted. The file is provided by the Commissioner for Human Rights.
- fluid-ui-settings (essential type) – stores information about the CSS style in which the CHR Website is displayed; it is stored on the User’s terminal device for 23 days or until manually deleted. The file is provided by the Commissioner for Human Rights.
Use of Data
The collected information (log files) helps us determine which subpages are most frequently visited, whether the website’s structure contains errors, which web browsers are used, etc. Based on this information, auxiliary statistics may be generated, which do not contain any characteristics that could identify the persons visiting the CHR Website. We analyze the log files to ensure the highest quality of the CHR Website.
This information is not disclosed to anyone except those authorized to administer the server and the CHR Office network, and only, when necessary, to entities entitled to receive it, solely on the basis of legal regulations.

The CHR Website contains links to other websites.
The Commissioner for Human Rights is not responsible for the privacy policies in force on those websites. It is advisable, therefore, after visiting other sites, to review their established Privacy Policy.
